The following government agencies have endorsed Docket(r), and commissioned it: Idaho Department of Health and Welfare; Minnesota Department of Health; New Jersey Department of Health; and Utah Department of Health.
Residents of Idaho, Minnesota, New Jersey and Utah who have a valid email address and/or phone number on file with their state’s immunization registry may access digital immunization records using Docket(r).
Docket(r), however, is completely optional.
Docket Health, Inc. works with public health to make it easy for you to view your personal and family immunization records. You can track your immunizations and review previous immunizations.
